Yardbird Suite (Suite) respects the privacy of all stakeholders. The Suite is committed to protect all personal information gathered and to use this information solely for the purpose for which it was collected.
Yardbird Suite adheres to all federal and provincial privacy, freedom of information and anti-spam policies, practices, regulations and related association practices and procedures as the foundation of this policy.
Principles
Personal information that may be collected could include (but is not limited to) any of the following:
- Donor: name, address, email, phone number, donation amount
- Patron: name, age, address, email, phone number
- Staff: name, address, email, phone number, sex, social insurance number, salary, tax related information, emergency contact information
- Volunteer: name, age, address, email, phone number, social insurance number, tax related information, emergency contact information
Withdrawal of Consent
Completion of any forms with personal information will imply consent to use the information for the purposes stated on the form. An individual has the right to withdraw consent of the use of their personal information at any time through a written, signed and dated request to the President of Yardbird Suite. Upon receipt of the written withdrawal of consent, the President will date when the withdrawal was received and place in the file (or electronically). The withdrawal request will be retained in accordance to the Yardbird retention policy.
Retention
Any forms or personal information in hard copy and electronic form will follow Yardbird Suite retention policy for retaining and destroying the information.
Oath of Confidentiality
Board, Staff and Volunteers of Yardbird Suite will be required to sign a yearly Oath of Confidentiality in order to be able to use any personal information for Yardbird purposes as outlined in this and other organizational policies.
Breach of Privacy Policy
Yardbird Suite is responsible for all personal information under its control, including any personal information. Should an individual feel that their privacy has been breached due to any Yardbird Suite practices, they may submit a written complaint (in accordance to the Yardbird Complaint Policy). Should an individual not be satisfied with the resolution of their complaint, they may address their complaint to the Alberta Privacy Office.
